In order to issue an I-20 we require the following documents:
Filled application form from Citi College of Allied Health
Copy of passport (Valid at least six months)
Two Photographs
Copy of high school diploma (Intermediate certificate)
Affidavit of Support on stamped paper or Form I-134
Bank Statement ($20,000.00 minimum)
TOEFL if requires
Application fees $150.00 (non-refundable)
SEVIS fees $200.00 (www.fmjfee.com)
